How It Works
Primary care designed for people who can’t afford delays
Concierge Care Center (CCC) is a membership-based primary care practice built around access, time, and proactive health management. You receive direct, physician-led care from Dr. Andre Small with the responsiveness and discretion that busy professionals expect.
Important: CCC does not accept or bill insurance.
1.
Start with a brief consultation A complimentary consultation confirms fit, answers questions, and helps determine which membership option best aligns with your goals and schedule.
2.
Complete onboarding and establish your baseline We begin with a structured intake that allows Dr. Small to understand your full health picture, including: - Medical history and current medications -Prior records (when available) -Current concerns, risks, and performance goals -A clear plan for prevention and ongoing monitoring
3.
Use care when you need it—without the usual friction Once you’re a member, you have a direct pathway to care for routine needs, urgent concerns, and ongoing management—without rushed appointments or long wait times.

This is primary care built around execution: clear decisions, fast follow-through, and continuity.

Insurance: How It Fits When CCC Doesn’t Bill It
CCC is a direct-pay practice. That means your membership is paid to CCC and is separate from insurance.
What your membership covers
Your membership covers the care relationship: access, evaluation, planning, follow-ups, and ongoing management as defined by your plan.
What insurance is still useful for
Many members keep insurance to cover higher-cost events and outside services such as:
- Hospital and emergency services
- Specialist visits and procedures
- Imaging (CT, MRI, ultrasound, X-ray)
- Surgeries and facility fees
CCC helps you navigate these situations and coordinate care—but insurance (or direct payment) typically covers the external costs.
A note for HSA / high-deductible plan strategies
For many entrepreneurs and business owners, a high-deductible or catastrophic plan combined with an HSA can be an effective structure. Eligibility rules vary, and membership fees are typically reimbursable through HSAs/FSAs. We recommend confirming with your plan administrator or tax professional.
Membership simplifies access to care, but certain costs are billed by third parties.
We will always aim for transparency so you can make informed decisions before you spend.
See services page for details.
CCC is designed to deliver premium primary care. It is not designed to replace emergency services or specialty care.
If you have symptoms that may be life-threatening—chest pain, stroke symptoms, severe shortness of breath, major trauma—call 911 or go to the nearest emergency room.
Membership improves access and continuity, but care recommendations are based on medical appropriateness and safety. Some concerns require:
- In-person evaluation
- Labs or imaging before treatment
- Referral to a specialist
You will have clear guidance on what is urgent versus non-urgent and how to reach the practice. Response times and availability can vary by plan, clinical volume, and the nature of the request.
Virtual care availability can be affected by state licensing rules. If you travel frequently or are often out of state, we’ll discuss how to plan around that.
